Triage RN

Responsibilities:

• Provide after-hours triage, clinical assessment, and symptom management via phone.
• Coordinate care with physicians, facilities, and internal teams.
• Accurately document all interactions and care plans in EMR.

Qualifications:

• Active RN license with no current or pending restrictions.
• Hospice experience and Home Care Home Base (HCHB) experience required.
