Core Responsibilities:
- Conduct thorough chart reviews for PACE applicants, evaluating medical records and clinical documentation.
- Present cases to Site Medical Directors and collaborate with Enrollment RNs to resolve gaps.
- Provide clinical expertise for Health & Safety determinations and support quality improvement initiatives.
Required Qualifications:
- Current Nurse Practitioner or Physician Assistant license in California.
- Minimum 3 years of post-licensure clinical experience in geriatrics or PACE-adjacent settings.
- Proficiency with geriatric assessment tools and Epic EHR.
Compensation and Benefits:
- Base salary range $116,000 - $152,000, with eligibility for annual bonus.
