Nurse Practitioner - National After-Hours Team - part time

Role Overview:

• As a member of our National After-Hours Call Team, you will provide compassionate telephonic care to residents in senior living communities during evenings, nights, weekends, and holidays.
• You will work remotely, serving as the trusted clinical partner for residents and facility staff during critical after-hours times.

Key Responsibilities:

• Serve as the first line of support, delivering high-quality, cost-effective care over the phone and via telehealth visits.
• Perform comprehensive assessments, document encounters in the EMR, and coordinate care with physicians and specialists.
• Manage high call volumes independently while providing coverage across multiple states.

Qualifications & Experience:

• Hold a Master's Degree as a Nurse Practitioner with active, unrestricted licensure in Texas and Louisiana (additional states preferred).
• Have at least 3 years of NP experience with a background in acute and chronic disease management, adult, family, or geriatrics.
• Possess strong computer skills and the ability to obtain DEA licensure and prescriptive authority.
