About the Role:
- Serve as the primary clinical point of contact for patients navigating their remote care journey.
- Lead clinical decision-making with a strong emphasis on guideline-directed medical therapy for heart failure.
- Collaborate closely with physicians and care teams to optimize treatment plans.
What You'll Do:
- Manage a panel of patients with timely clinical decisions based on evidence-based guidelines.
- Initiate and titrate GDMT while collaborating with physicians and RNs.
- Respond promptly to clinical escalations from remote patient monitoring.
What You Need:
- 5+ years treating patients with chronic diseases in a high-performing medical center.
- 3+ years of cardiology experience with focus on heart failure management.
- Active ANCC or AANP certification and willingness to expand state licensure.
