Job Description

The Nutrition Instructor will provide telephonic nutrition education to members with chronic conditions as they learn to select, prepare, and eat foods that are appropriate for their condition(s) as they participate in a chronic care meals program. Chronic conditions can include, but are not limited to diabetes, heart disease and kidney disease. Utilizing a member-centric approach, the Nutrition Instructor will collaborate with the member to explore the nutrition-related issues and topics that are most important to the member. The Nutrition Instructor will also help connect members to other nutrition and condition resources available from the health plan or system or in their community. Responsibilities may include, but are not limited to Provide telephonic nutrition education sessions per member; number of sessions offered varies, based on what program the member is enrolled in Document goals and care plan from education sessions in platform Schedule follow-up calls with each member/patient Identify community resources for members as needed Maintain and update professional knowledge by participating in educational opportunities; reading professional publications; maintaining personal networks; participating in professional organizations
