The Nutrition Instructor will provide telephonic nutrition education to members with chronic conditions as they learn to select, prepare, and eat foods that are appropriate for their condition(s) as they participate in a chronic care meals program. Utilizing a member-centric approach, the Nutrition Instructor will collaborate with the member to explore the nutrition-related issues.
