Bilingual Community Health Worker (Remote Flexible, Spanish Speaking)

As a Bilingual Community Health Worker, you will:

• Serve as a primary point of contact for a panel of members, maintaining consistent phone, text, and in-person outreach
• Build long-term, trust-based relationships to support engagement and continuity of care
• Meet outreach, documentation, and engagement targets in line with program requirements

Your responsibilities include:

• Helping members navigate healthcare and social service systems, including housing, food access, transportation, and behavioral health
• Coordinating care activities, such as scheduling appointments, managing referrals, and ensuring timely follow-ups
• Collaborating closely with nurses, behavioral health care managers, and other care team members to ensure high-quality, integrated support

You need:

• Active CHW Certification in the state of Texas (required)
• Bilingual in English and Spanish (required)
• Strong communication skills and cultural competency when working with vulnerable populations
