About the Role:
- The Behavioral Health Care Manager provides care coordination and behavioral health support under clinical oversight.
- You will implement the Collaborative Mental Health Care (CoCM) program for patients and caregivers.
Minimum Qualifications:
- Active independent clinical license (LCSW, LMFT, LPC, or equivalent) with ability to practice independently.
- 0-2 years of clinical case or care management experience.
- Strong organizational, communication, and interpersonal skills.
