About the Role:
- Provide evidence-based interventions via Collaborative Care Model to individuals (pediatric and/or adult) experiencing mild to moderate symptoms.
- Screen patients for behavioral health and substance use disorders using tools like PHQ9, GAD7, and Columbia Suicide Risk Assessment.
- Coordinate closely with primary care providers and other service providers to ensure integrated care.
Responsibilities:
- Monitor patients for changes in clinical symptoms and treatment side effects, tracking outcomes using a registry.
- Plan specific self-management goals and facilitate care plan changes for patients not improving.
- Educate patients about behavioral health and substance use disorders and treatment options.
Qualifications:
- Master’s Degree in Social Work, Counseling, or related field from an accredited program.
- Licensed to practice in California as an independent clinician (LCSW, LPCC, or equivalent).
- Proficient in medical interpretation and documentation in English and Farsi.
