About the Role:
- As a Nurse Practitioner, you will provide high-quality follow-up care to post-hospital discharge patients via phone and video.
- You will coordinate care across settings and collaborate with a nursing team to achieve population health goals.
About You:
- You are a culturally-competent clinician with strong communication and motivational interviewing skills.
- You have 5+ years of clinical experience, preferably in primary care or geriatrics.
- You are comfortable in a digital-first environment and proficient with technology.
