Virtually Integrated Care RN

Job Summary:

• As a Virtually Integrated Care RN, you will empower patients toward optimal health through comprehensive remote support and collaborative care coordination.
• You will assess patient needs, develop personalized care plans, and provide direct clinical coaching in a dynamic virtual environment.

Responsibilities:

• Collaborate virtually with physicians, nurses, and health care team members in the provision of clinical care.
• Assess medical, physical, and psychological status of patients by reviewing records and diagnostic studies.
• Educate patients and families on disease processes, procedures, and self-care to maximize wellness upon discharge.

Qualifications:

• Required: Bachelor of Nursing (or actively pursuing) with minimum 3 years acute care nursing experience.
• Required: Registered Nurse license in Arizona and Basic Life Support certification.
• Preferred: Master of Nursing or Clinical Nurse Specialist certification.
