Registered Nurse

Play a pivotal role in the Clinical Care Delivery Team as a Registered Nurse. You'll be instrumental in delivering top-notch, evidence-based care to a panel of seriously ill patients through telemedicine. Manage patients assigned to your Care Team, ensuring visit frequencies align with patient needs. Communicate with patients via phone and video conferencing. Provide patient care within your scope of practice and per provider orders. Assess and enroll patients, develop and update care plans based on ongoing assessments. Manage patient medications and facilitate medical supplies. Coordinate with the Interdisciplinary team and external agencies, including advanced care planning and hospice referrals. Educate patients and caregivers on services, disease processes, symptom management, and medication tracking; and perform additional duties as assigned.
