Job Description
Manage the CCC program to meet national patient access goals and improve engagement. Create and implement consistent training for CCCs to enhance service quality. Establish and track KPIs for CCCs, using data to drive improvements. Streamline referral and onboarding processes to enhance patient access. Work with clinical and operational leaders to align the program with patient and partner needs. Analyze data to inform decisions and improve patient access strategies. Guide CCCs in building local healthcare partnerships to increase referrals. Maintain program compliance with Hanger Clinic’s care standards.
