Job Description
The Care Coordinator will be responsible for patient support, care coordination, and emotional guidance. You will help each new patient navigate the journey into care, from matching with the right therapist to feeling confident and understood every step of the way.
You will be an Empathetic and clear communicator, listening actively, and making others feel at ease. This role requires someone who is Highly organized and detail-oriented and tech-comfortable to learn new tools. This work helps older adults and their families feel seen, supported, and cared for helping to bring life-changing care to a population that’s too often overlooked.
