Responsibilities:
- Conduct telephonic and in-home assessments of medical, psychosocial, physical, and spiritual needs.
- Develop, implement, and monitor care plans, ensuring continuity of care.
- Monitor members' chronic care benefits and coordinate interdisciplinary services.
Qualifications:
- BS in social work or related field required; Master's preferred.
- Current NY State CSW preferred for Master's candidates.
- Prior human services experience and strong assessment and crisis intervention skills required.
