Role Responsibilities:
- Managing the overall care management of the patient panel.
- Establishing trusting and empathetic relationships with patients.
- Serving as an advocate and community liaison for patients.
Required Qualifications:
- Associate degree in nursing.
- Current RN License is required, Compact License preferred.
- Care management experience required.
