Role Responsibilities:

- Schedule pharmacist consultations and coordinate follow-up.
- Serve as a liaison between patients and pharmacies.
Required Qualifications:
- High school diploma or equivalent.
- Strong interpersonal and communication skills.
- Experience in healthcare or care coordination.
