Key Responsibilities:
- Patient Engagement & Orientation: Conduct IOP orientations for new patients.
- Care Coordination & Discharge Planning: Lead discharge planning beginning at admission.
- Indirect Patient Contact (IPC) & Compliance: Document collateral contacts, discharge coordination, and case management activities.
Required Qualifications:
- 3-5 years of experience working as a Clinical Case Manager.
- Current clinical licensure in the state of California.
- Proficiency with telehealth platforms and remote clinical work.
