Primary Duties:
- Counsel patients on medication adherence and address medication-related problems.
- Perform clinical chart reviews to inform patient interventions.
- Escalate complex clinical cases to prescribers as needed.
Clinical Innovation:
- Identify gaps in medication management and translate insights into recommendations.
- Participate in the design and evaluation of pilot programs.
- Serve as a clinical subject matter expert for organizational objectives.
Minimum Qualifications:
- Doctor of Pharmacy (PharmD) and active pharmacist license.
- Experience in value-based care or community pharmacy.
- Strong communication skills and proficiency in EHR systems.
