Responsibilities:
- Review and validate all assigned assessments for accuracy, completeness, and internal consistency.
- Correct errors that affect reimbursement, quality measures, or compliance before submission.
- Communicate promptly with clinicians and leadership about discrepancies and process improvements.
Requirements:
- Active, unrestricted LPN or LVN with a minimum of 2 years of home health experience.
- Strong understanding of Medicare home health reimbursement and PDGM.
- Proficiency with EMR systems and Microsoft Office; HCHB preferred.
