As a Patient Engagement Specialist, you'll support patients and clinical care teams, playing a key role in ensuring patients engage with Evergreen Nephrology and their local care teams. You will partner with market operations teams across the US to ensure patients are actively engaging in their care, working with their interdisciplinary care team, and achieving their kidney care and overall care goals. You will have the opportunity to work closely with Senior Leadership at Evergreen to build the foundation of our contact center.
Responsibilities include managing a high volume of outbound and inbound new patient onboarding calls, building trust with patients and clinical care team members through exceptional omnichannel customer service, and actively listening during patient conversations to activate personalized support. You'll also assist with patient and practice administrative requests and contribute to building new workflows related to contact center operations. This is a remote position, requiring candidates to work 9:00a-5:30p CT.
