Responsibilities:
- Facilitates ongoing staff development and acts as mentor
- Demonstrates oversight of care management activities and evaluates performance metrics
- Identifies and implements plans to achieve results
Qualifications:
- At least five (5) years’ experience as a care manager
- Supervisory/leadership experience preferred
- Registered Nurse or Master’s in Social Work degree
Knowledge:
- Keeps abreast of developing trends and core leadership/management skills
- Knowledge of NCQA standards for Population Health Management
- Knowledge of clinical and managed care principles and operations
