Ennoble Care is seeking an experienced Full-time Behavioral Health Specialist for our department supporting patients within and around the Hoboken / Jersey City, NJ area. This position is primarily remote, with the possibility of occasional in-person visits in the future based on patient needs. The Social Worker Care Manager will support the mental and physical healthcare of patients on an assigned patient caseload, screen and assess patients for common mental health and substance abuse disorders, and facilitate patient engagement and follow-up care.
