Be at the forefront of their mission to make therapy accessible for all. You will be responsible for fulfilling requests and supporting administrative responsibilities for their clients referred through Veterans Affairs, Health Systems, and various partnerships. In this role, you will also work with external partners and healthcare systems and utilize various technologies to ensure the client is connected and maintains the required support through an episode of care.
You will interact with external partners, healthcare systems, and clients to ensure access to care for all SonderMind clients. You will facilitate care coordination activities such as lead and task management, referral processing, phone calls, and post-session documentation and reporting.
You'll also assemble and maintain accurate and appropriate documentation for referred clients throughout their mental health journey. You will manage inbound and outbound calls and other forms of communication while applying standard guidelines to address or route messages to the appropriate care team member for follow-up when necessary. Ensures all referrals are complete and are processed in a timely and accurate manner via EMRs and other referral sources. You will also conduct outreach to referred clients and partners to obtain critical information to aid in the journey to care and other responsibilities and ad-hoc projects from time to time based on business needs.
