About the Role:
- Serve as a key advisor, engaging with members to support their health care needs.
- Create and manage care plans to ensure health care goals are met.
- Partner with members, families, providers, and community resources to coordinate care.
Qualifications:
- Completion of an accredited nursing program.
- Active and unrestricted RN License in Iowa or South Dakota.
- Certified Case Manager (CCM) required within 24 months of hire.
Responsibilities:
- Utilize critical thinking to recognize signs and symptoms of potential high-risk conditions.
- Proactively identify barriers and gaps to care while managing care plans.
- Document care management activities accurately and timely using technology tools.
