Licensed Therapist (LCSW, LMFT, LPCC)

Key Outcomes:

• Deliver evidence-based therapeutic interventions (e.g., CBT, Motivational Interviewing) to older adults dealing with depression, anxiety, life transitions, chronic pain, or behavioral barriers to health.
• Partner with patients to build sustainable routines around stress management, sleep hygiene, nutrition, physical activity, and weight loss.
• Use EMR for documenting patient information and communicating to other health care providers.

Required Qualifications:

• Active and unrestricted LCSW, LMFT, LPCC license in multi-states including CA, TX, FL, and/or PA.
• 3+ years of clinical experience and 1+ years in telehealth with high proficiency in EMRs.
• Prior experience working with older adults (65+) or in geriatric health, chronic disease management, or lifestyle/preventative medicine.
