Provide evidence-based interventions via the Collaborative Care Model to individuals with mild to moderate symptoms.
Work with pediatric and/or adult populations to promote symptom reduction and optimal patient outcomes.
Report to the Clinical Excellence Manager and collaborate with a care team.

Responsibilities:

Screen patients for common behavioral health and substance use disorders using PHQ9, GAD7, and Columbia Suicide Risk Assessment.
Monitor patients for changes in symptoms and treatment side effects, and support medication management prescribed by PCPs.
Track patient outcomes using a registry and document progress in Salesforce and the electronic health record.

Compensation & Benefits:

Base salary ranges from $56,707.60 to $102,585.10 depending on licensure and geographic location.
