Job Description
Responsibilities:
- Maintain ongoing caseload of individuals using evidence-based approaches.
- Conduct telephonic and SMS outreach to ensure timely follow-up to members.
- Identify and break down barriers ensuring individuals’ continuation with the program.
Requirements:
- 1+ years of general work experience, preferably in Case Management.
- Strong understanding of cultural fluency and high degree of empathy.
- Strong technical skills and comfort with technology innovation.
