Bilingual Case Manager (Remote Flexible, Spanish Speaking)

Responsibilities:

• Maintain ongoing caseload of individuals using evidence-based approaches.
• Conduct telephonic and SMS outreach to ensure timely follow-up to members.
• Identify and break down barriers ensuring individuals’ continuation with the program.

Requirements:

• 1+ years of general work experience, preferably in Case Management.
• Strong understanding of cultural fluency and high degree of empathy.
• Strong technical skills and comfort with technology innovation.
