Position Summary:
- Managing all patient care coordination
- Caregiver-related issues that arise outside of standard business hours
- Ensuring continuity of care for Home Health Aide (HHA) and Private Duty Nursing (PDN)
Key Responsibilities:
- Responding promptly to incoming calls related to staffing, attendance, patient needs, and caregiver issues
- Monitoring and verifying caregiver attendance for all scheduled HHA and PDN cases
- Documenting all calls, incidents, and case-related updates
Working Conditions:
- Managing high call volumes, urgent staffing needs
- Time-sensitive documentation outside of regular office hours
- High call volumes during nights and weekends
