Role Overview:
- Provide remote, patient-centered nursing support to individuals with chronic conditions.
- Interpret biometric and IVR data to identify clinical changes and develop care plans.
- Collaborate with patients, caregivers, and interdisciplinary teams to improve outcomes.
Key Responsibilities:
- Conduct telephonic nursing assessments and adjust care plans based on patient needs.
- Use Motivational Interviewing to encourage healthy behavior changes.
- Coordinate care transitions and escalate urgent concerns per clinical guidelines.
Requirements:
- Associate or Bachelor’s degree in Nursing with active RN license.
- At least 5 years of clinical nursing experience and 3 years in care management or telehealth.
- Strong clinical assessment, communication, and EHR skills.
