- Evaluate patient safety reports using standardized systems of classification.
- Facilitate investigation and action-planning for events of harm, precursor events, and near misses.
- Conduct Apparent Cause Analyses and support Root Cause Analyses.
Proactive Risk Assessment & System Reliability:
- Lead or support Failure Mode and Effects Analyses to identify and mitigate risks.
- Conduct gap assessments and design system-level actions to address vulnerabilities.
- Apply reliable design principles and safety science practices to improve care processes.
Culture of Safety & Learning:
- Support completion and analysis of the Culture of Safety Survey.
- Promote harm reporting through a non-punitive approach.
- Foster a just, restorative culture by supporting fair accountability.
