Who We Are:
- Imagine Pediatrics is a tech-enabled, pediatrician-led medical group dedicated to children with special health care needs.
- We deliver 24/7 virtual and in-home care, enhancing existing care teams with compassion and creativity.
What You'll Do:
- Provide direct support to families addressing social needs and closing gaps related to non-medical drivers of health.
- Build and strengthen community relationships, maintaining a robust resource network.
- Offer non-clinical support and education to families navigating complex medical and behavioral health challenges.
What You Bring:
- Bachelor's degree in healthcare, behavioral health, or social services preferred.
- Certification as a Community Health Worker highly desired.
- Familiarity with navigating social service agencies and experience with telephonic services.
What We Offer:
- Hourly rate $25-30/hr plus competitive benefits.
