Position Objective:
- Providing medication management services for medically and socially complex patients.
- Managing opportunities for professional development within the pharmacy department.
- Participating in patient intervention and referral, risk management, and ethical decision-making.
Responsibilities:
- Develop and maintain protocols for prescribing, dispensing, and administering medications.
- Contribute to the efficient delivery of care by participating in quality improvement initiatives.
- Participate in the development or revision of pharmacy policies and procedures.
Position Requirements/ Skills:
- A doctor of pharmacy degree from an accredited institution and licensure as a pharmacist in the state of practice.
- Proven experience as a clinical pharmacist.
- Ability to manage multiple priorities in an expedient and decisive manner.
