Job Description
Salvo Health is seeking an experienced LCSW/LICSW to play an integral role on our care team. This role will work directly with patients, offering personalized psychosocial support and counseling to patients to reduce symptoms and improve quality of life. The LCSW will empower patients with the knowledge and tools to make lasting behavioral changes that will improve clinical outcomes and overall wellness. Responsibilities include conducting comprehensive psychosocial assessments, providing counseling and health education, and developing personalized care plans in collaboration with the interdisciplinary healthcare team. The LCSW will also offer crisis intervention, create consumer-facing mental health content, and work cross-functionally to improve the in-app patient experience. Candidates should have an LCSW license, 2+ years of experience, and proficiency in telehealth platforms and EHR systems.
