About the Role:
- Conduct virtual wellness visits and health assessments for patients of all ages.
- Utilize modern technology to navigate schedules and coordinate with in-home Sprinters.
- Provide patient-centric care, including health risk assessments and medication reviews.
What We're Looking For:
- Board Certified Family Nurse Practitioner with an active Louisiana license.
- Strong problem-solving skills and the ability to work independently.
- Technologically savvy with excellent written and verbal communication.
