YOUR ROLE:
- See members for specialty palliative care outpatient appointments via video visits as part of an interdisciplinary team.
- Oversee and direct interdisciplinary case conferences for members with advanced serious illness.
- Provide education about palliative care to the care team, focusing on symptom management and advance care planning.
WHAT LEADS TO SUCCESS:
- At least 3 years of nurse practitioner experience with 2 years in palliative care, preferably outpatient oncology.
- People-first mindset and effective listening and communication skills.
- Comfort with ambiguity and a desire to learn new technologies.
