Using a telemedicine platform, evaluate and manage members with urgent symptoms, complications, or exacerbations, developing comprehensive care plans and triaging to appropriate care.
Develop care plans for members in collaboration with onboarding RNs, addressing clinical and non-clinical needs, and prioritizing goals with concrete plans for the care team.
Oversee interdisciplinary case conferences, providing clinical oversight and direction to the case management team, and directing clinical care as needed.

Qualifications:

At least 3 years of NP experience, including 2 years in adult oncology with urgent care and 1 year of telehealth experience.
Unrestricted license with willingness to obtain additional state licenses; compact RN license preferred.
Experience at a startup or tech-forward company is exceptional.
