Responsibilities:
- Perform comprehensive clinical assessments and oversee member health throughout the cancer journey.
- Address clinical issues, manage escalations, and facilitate end-of-life care discussions.
- Coordinate care with providers, payers, and non-clinical team members to address social determinants of health.
Qualifications:
- Active, unrestricted compact RN license required; BSN preferred.
- At least 5 years of nursing experience including 3 years in solid tumor oncology.
- Oncology certifications (OCN, AOCN, AOCNS, or CCM) required or obtained within 2 years.
