Clinical Care Coordinator

Key Responsibilities:

• Communicate with patients regarding appointments, treatment plans, and follow-ups.
• Coordinate patient care between providers, specialists, and insurance companies.
• Maintain accurate and confidential patient records in EHR/EMR systems.

Qualifications:

• Clinical background as a Nurse, Doctor, or Medical Assistant.
• Strong understanding of medical terminology and clinical workflows.
• Experience in care coordination or virtual medical support.

Preferred Experience:

• Chronic Care Management or prior authorizations.
• Insurance verification and referral coordination.
• EHR/EMR documentation and patient intake.

Ideal Candidate:

• Combines clinical knowledge with strong administrative and communication skills.
• Comfortable working independently in a remote environment managing multiple patients.
• Understands HIPAA and ensures timely, well-coordinated care.
