Lead Care Manager

We're seeking an exceptional Lead Care Manager (LCM) to join our team, serving as the client’s primary point of contact and working with all their providers. The LCM manages client cases, coordinates health care benefits, provides education and facilitates member access to care. Responsibilities include assessing member needs, overseeing the development of care plans, connecting clients to social services, advocating on behalf of the client with health care professionals. The LCM will also utilize evidence-based practices, conduct outreach activities, evaluate client’s progress, provide mental health promotion, and complete documentation. This role requires working Monday-Friday 8:30am-5:00pm Pacific Time and requires 2+ years experience as a care manager.
