About the Role:
- The Medical Records Specialist works with the Care Navigation Team to improve efficiency and quality of care by ensuring that all patient medical records are readily available in an organized, easily accessible format.
- This role is pivotal in facilitating new patient referrals and supporting the ongoing client journey.
What You'll Do:
- Facilitate new patient referral intake process by acquiring, organizing, and summarizing medical records from all providers.
- Build strong working relationships with the Care Navigation Team, including Care Navigator and Hospital Logistics Expert.
What You'll Bring:
- 5+ years of experience in healthcare, including veterinary technician or nurse roles.
- High proficiency with Electronic Medical Record/PIMS systems and strong attention to detail.
