Intake Coordinator - PRN

Responsibilities:

• Plan, direct, and evaluate potential patients through the assigned referral system.
• Establish consistent communication with admitting hospitals for a best-in-class admission process.
• Work with Outreach Managers to assess patient needs using the Pre-Screening Evaluation Form.

Qualifications:

• Associate's degree in social work, psychology, counseling, human services, nursing, or public health required.
• Current and active LPN or RN license in state of residence preferred.
• Minimum of 1 year experience in acute psychiatric admissions preferred.
