RN Program Manager

As Program Manager, you will manage a team dedicated to providing longitudinal care management for patients with kidney disease. You will serve as a collaborative and supportive leader responsible for the day-to-day operations of your care management team, focusing on proactive, person-centered care. Your primary duties include managing the performance of team members, providing ongoing training and development, and ensuring adherence to best practices and protocols. You will also be responsible for overseeing the daily operations of your team, developing and implementing collaborative strategies, completing routine audits, preparing performance reports, fostering relationships with stakeholders, and continuously evaluating and improving the program's performance.
