Job Description

Care Management Duties:

  • Assess member needs in physical and mental health, SUD, palliative care, and other areas.
  • Oversee development of client care plans and goal settings.
  • Connect clients to needed social services and supports.

Qualifications and Experience:

  • Must work Monday-Friday 8:30am-5:00pm Pacific Time, with possible evenings/Saturday mornings.
  • Requires 2+ years of experience as a care manager, care navigator, or community health worker.
  • Must have general computer skills and a working knowledge of Google Workspace.
