Deliver compassionate after-hours care virtually, primarily by phone, to aging residents across multiple states.
Act as the trusted voice and clinical partner for residents and facility staff during evenings, nights, weekends, and holidays.

Essential Duties:

Serve as first-line support and direct medical care over the phone using Curana's telephonic platform.
Perform comprehensive assessments, document in EMR, and apply clinical protocols to support safe treatment in place.
Collaborate with physicians and specialty providers to address acute, chronic, and behavioral health needs.

Qualifications:

Master's Degree as a Nurse Practitioner with current unrestricted NP license in Pennsylvania and New York.
Nurse Practitioner national certification as ANP, FNP, or GNP and ability to obtain DEA licensure.
3+ years of experience as an NP with background in acute and chronic disease management.

Scheduling:

Part-time role with 24 hours per week, including two 6-hour weeknight shifts and every other weekend day shifts.
Overnight and holiday coverage are required, with 2 holiday shifts per year for part-time team members.
