Hospice Triage RN - Remote Field Staff - Supportive Care

Role Overview:

• Serve as a key member of the interdisciplinary team providing after-hours clinical support to patients and families.
• Work fully remote using Telecare protocols to assess needs and coordinate care.

Key Responsibilities:

• Perform triage, clinical assessment, and interventions via phone.
• Support patients and families with symptom management and end-of-life care needs.
• Collaborate with physicians, facilities, and internal teams to ensure positive outcomes.

Qualifications:

• Active RN license with no current or pending restrictions.
• Hospice experience and familiarity with Home Care Home Base (HCHB) software.
• Strong clinical judgment, critical thinking, and communication skills.
