About the Role:
- We are seeking a dedicated Registered Nurse to join our Clinical Care Delivery Team.
- You will provide top-notch, evidence-based care to seriously ill patients through telemedicine.
Key Responsibilities:
- Manage patients assigned to your Care Team, ensuring visit frequencies align with acuity.
- Communicate with patients via phone and video conferencing as per care plan.
- Coordinate care with interdisciplinary team and external agencies.
Qualifications:
- Active, unrestricted California RN license; willingness to obtain licensure in other states.
- 1+ year experience in palliative or hospice care.
- Excellent oral and written communication skills.
