Triage RN Remote Field Staff – Supportive Care

Role Responsibilities:

• Provide after-hours triage, clinical assessment, and interventions via phone.
• Support patients and families with symptom management and end-of-life care needs.
• Dispatch visits and coordinate services with vendors as needed.

Qualifications:

• Active RN license with hospice experience required.
• Strong clinical judgment, critical thinking, and phone-based communication skills.
• Experience with Home Care Home Base (HCHB) preferred.

Compensation & Benefits:

• Salary range: $70,000 - $72,800 per year.
