Clinical Pharmacist, Pharmacy Outreach

Primary Duties:

• Counsel patients via telephonic outreach on medication adherence and make evidence-based recommendations.
• Perform clinical chart reviews, document patient commitments, and escalate complex cases.
• Collaborate with teams to optimize workflows and meet performance goals.

Minimum Qualifications:

• Doctor of Pharmacy (PharmD) with active multi-state licensure.
• Minimum of 3 years of experience in value-based care, ambulatory care, or community pharmacy.
• Strong verbal and written communication skills and proficiency in EHR systems.
