Job Description
Responsibilities:
- Assessing member needs in physical, mental health, and social supports.
- Overseeing the development of client care plans and goal settings.
- Offering services via telehealth or field-based services.
Required Experience:
- Active LCSW, LMFT, or LPCC license in California is required.
- 1+ year of experience as a care manager.
- Working knowledge of government and community resources related to social determinants of health.
Skills:
- Excellent oral and written communication skills.
- Positive interpersonal skills.
- General computer skills with Google Workspace and MS Office.
